Tokyo Governor Yuriko Koike, aged 71, is set to win re-election with over 40% of the vote, according to an exit poll.
Her closest competitor is Shinji Ishimaru, 41, a former mayor of a town in Hiroshima prefecture, who came in second place.
Renho, 56, backed by the Constitutional Democratic Party of Japan (CDPJ), finished third.
Koike's victory could bolster embattled Japanese Prime Minister Fumio Kishida amid declining public support and a political donations scandal.
